Your TRT prescription, sent to your pharmacy
If TRT is right for you, Dr. Ibrahim sends your prescription straight to your pharmacy — sometimes the same day.
TRT is prescribed only when medically appropriate. Some patients may not be candidates.
You're in the right place if any of these sound like you:
You've been told your labs are "normal" — but you know something is wrong.
Your testosterone was never fully evaluated. You were told it was stress, aging, depression, or that you just needed to exercise more.
You've done the lifestyle work — gym, sleep, diet. You're still tired, foggy, and flat.
You're currently on Testosterone Replacement Therapy (TRT) with another provider and feel like a number.
You want a real physician to give you an honest opinion before committing to anything.
What this consultation is
You'll get my honest opinion. If TRT isn't right for you, I'll tell you. If it is, I'll explain what I'd recommend — and you're free to take that opinion to any provider you'd like.

Start treatment, often right away
If TRT is right for you, Dr. Ibrahim can prescribe it and send it straight to your pharmacy.
This is the same 30-minute physician consultation that costs $250–350 at most California specialty practices. It's the actual visit — not a sales call before the visit. We're offering it free while we grow Optimal State Health's California practice.
What this is not
This is not a sales call.
This is not a prescription guarantee.
This is not an automated portal.
This is not a trap of hidden fees.
This is not a one-size-fits-all TRT protocol.
This is not a pre-screening before the real visit — this is the visit.
It's a real consultation with a physician who will tell you whether Testosterone Replacement Therapy (TRT) does or does not make sense for you.
No credit card. No obligation.

This is how I practice
“When you're working with me, you get my opinion when you need it. If labs come back between visits, I review them. If your protocol needs adjustment, I explain why. If a question can be handled without a full appointment, we handle it directly.
That's the difference between being managed by a physician and being processed through a system.
If we decide to work together
The consultation is free. There's no obligation to continue after it. If TRT is appropriate and you want Dr. Ibrahim to manage your care, here's what that looks like:
$149/month includes:
- Follow-up visits with Dr. Ibrahim
- Lab review and protocol adjustments as needed
- Direct messaging between visits
- Testosterone prescription sent to your pharmacy
Paid separately at cost — not through Optimal State Health:
- •Labs, paid directly to the lab. Cash pricing is typically $80–150 every 3–6 months; insurance may cover in some cases.
- •Medication, paid through your pharmacy. Cash pricing is typically $20–50 per month; insurance may cover in some cases.
This is the cost of ongoing physician-led care: the same doctor who knows you, manages your care directly, and is reachable when you need answers.
No setup fee. No hidden charges. No contract. Cancel anytime.
Common questions
No problem. We can start by talking through your symptoms, history, and goals. If updated labs are needed before making any treatment decision, I'll explain exactly what to order and why. Labs are paid directly to the lab, not to Optimal State Health.Depending on your insurance and clinical situation, labs may be covered by your plan.
That happens, and in many cases, it's the actual outcome. If TRT isn't right for you, I'll tell you what I think is going on instead — sometimes it's a sleep issue, sometimes thyroid, sometimes lifestyle, sometimes something I'd want you to follow up on with your primary care or a specialist. You'll leave the call with a clear sense of what to do next. No prescription, no charge, no obligation to come back.
Three things, mainly. First, your consultation is directly with Dr. Ibrahim — not a sales rep, not a rotating provider, and not an automated protocol. The doctor you book with is the doctor you’ll work with. Second, the consultation is the actual visit, not a sales call before the visit. Third, if you decide to continue care after the free consultation, pricing is transparent: $149/month for ongoing physician-led care, with labs and medication paid separately at cost. No setup fee, no hidden charges, no contract. contract.
Yes — this is important and worth taking seriously. Standard testosterone therapy can suppress sperm production, sometimes significantly. If you're actively trying to conceive or planning to in the next 6–12 months, then I don't typically recommend starting TRT — there may be other approaches that better support fertility goals. The right path depends on your specific situation, and that conversation matters before any treatment decision.
In most cases, yes — for the question of whether TRT is worth considering. A focused 30-minute conversation with someone who specializes in this is more diagnostic than a 7-minute primary care visit where hormone questions are one of many topics. If labs or additional evaluation are needed, I'll tell you before recommending anything.
You're welcome here. Bring your current protocol, your most recent labs, and any concerns about how things are going. I'll give you my honest opinion on whether your current approach makes sense, what I'd adjust, and whether switching providers is worth it for your situation. If your current setup is working well, I'll tell you that too.
Currently, we only serve patients in the state of California. We don't prescribe or manage care for patients outside the state. If you live elsewhere, I'd recommend looking for a board-certified physician licensed in your state with a similar practice approach.
Sometimes. Labs and medication may be covered by insurance depending on your plan, baseline labs, diagnosis, and the specific testosterone product. Many patients also choose cash-pay options because they can be straightforward and affordable — often around $20–50/month for medication. We can discuss cost-effective options during your consultation.
You can cancel anytime, no questions asked. There's no contract, no cancellation fee, no notice period. If you cancel and want to return later, we'll review where things stand and decide the appropriate next step.
Timing depends on appointment availability, whether labs are already completed, and how quickly lab results return. If TRT is appropriate and labs are complete, the process can move quickly. If labs are needed, lab turnaround is often several days. Most patients who are good candidates are on treatment within 1–2 weeks of their initial consultation.
